Thanks for this update Dave. My hepatologist is considering starting me on Fenofibrate since I seem to have plateaued on Urso. I’ll try bumping my urso up slightly before going down the Fenofibrate route. Moreover, it’s helpful to see someone that has had prolonged elevated ALK levels. I’m in the same boat with an elevated ALK since 2014. Aside from a big spike associated with an UC flare, I haven’t had any issues. I only received the small duct PSC dx after a second reading of my liver biopsy in 2019. MRIs and fibroscans have been normal and I haven’t had a PSC-related symptom to date. Still, I feel like my liver is a time bomb waiting to go off. Your story helps give hope that a transplant may be way in the future, or perhaps not at all…other stories on the board help understand that if a transplant is ultimately in the cards, it’s not the end of the world! Thnanks for posting Brian, and let me know what you and your Dr. decide and how it goes. If you do try fenofibrate I would be interested in whether your Dr. would want to keep you on Urso or discontinue it.
