A patient with PSC/UC who responds to oral vancomycin may not need any other medications for his liver or colon. Symptoms and progression will return if you stop treatment. There are quite a few published papers about the efficacy of this treatment. If you are interested I can provide them to you. Have him stay on vancomycin! My daughter takes 2000mg/day and has for 8 years. She has a normal liver and normal colon.

The important thing is that your son has a diagnosis; whether small- or large-duct probably has little role in terms of treatment plan. Per the definitions in the ACG and AASLD Clinical Guidelines, it appears that your son has classic large-duct PSC as he was diagnosed via ERCP (though affecting the smaller intrahepatic ducts).
ACG:
βSmall duct PSC makes up Λ5% of the entire spectrum of patients with PSC. The macroscopic bile ducts in small duct PSC are normal; hence, cholangiography [ERCP] will not be revealing. Diagnosis of small duct PSC requires liver biopsy, to demonstrate the characteristic microscopic disease. Interestingly, a number of patients with small duct PSC may later develop classic bile duct changes of large duct disease. The risk of progression from small duct PSC to classic disease is unknown.β

Hi everyone.
Is it common to get pancreatitis and elevated pancreas enzymes together with PSC?
My son had pancreatitis twice and his amylase and lipase are elevate every time with liver enzymes together like they fallow each other?
