I belong to an Awesome FB Support Group for PSC, called PSC Partners Seeking a Cure\_ and this survey was posted. Have permission to post here and hoping to keep the data coming.  
Also highly recommend connecting with this group. There are a couple on FB that are FABULOUS also.

We need your participation! Brigham & Women’s Hospital and Harvard University researchers have been trying to recruit 400 PSCers to complete a simple nutrition survey for a nutrition study we all would want to know the results to. We all want to know the relationship of nutrition and PSC! To date, they only have 200 completed surveys. Here is our chance to help them! If you attended our last two conferences, you probably completed their simple survey. If you live in the US, have PSC and are 18 years old and over, you can complete this simple task. It will take a few minutes of your time to allow these researchers to start their study! Here is the link to the survey.

Alternatively – they say that you can opt out of the gift card, and the $20 will become a donation to PSC Partners.
